Orbis Education hosts professional learning events throughout the year, including the Engagement in Healthcare Education Conference (the “Conference”). By registering for the Conference, you consent to the creation and maintenance by Orbis of a web profile containing certain personally identifiable information (“Information”), and Orbis will access this Information to provide the individual user with information and services associated with the event. In addition, during the Conference registration process, Orbis collects and will collect voluntarily shared personal data regarding accessibility and dietary requests.

Orbis will collect information about speakers and presenters at the Conference, including name, organization, and contact information. Orbis may also collect information provided by event attendees who evaluated a speaker’s or presenter’s presentation and associate it with the speaker’s record in a database. In certain instances, and with the speaker’s approval, Orbis will make and store a recording of the speaker’s or presenter’s presentation or materials. Orbis may share basic registration information, including name, title and institution name of a participant at particular events (e.g., webinar or preconference workshop) with the designated speaker(s) or presenter(s) for the purpose of planning for the event and tailoring the presentation to the group.

As is common practice on most websites, Orbis collects certain information automatically on this Orbis owned website and stores it in log files. As individual users interact on this Orbis owned website, Orbis may collect information about the user’s computer or device, and his or her visits to this Orbis owned website (“automatically collected data”) through cookies, Internet Protocol (IP) address tracking/URL tracking and other tools (collectively referred to as “tracking technologies”). The types of automatically collected data on this Orbis owned website through the use of these and other tools include: the search terms used, new or returning user information, browser information, computer or device type, operating system, internet service provider, website usage, referring/exit pages, platform type, date/time stamp, number of clicks and ads viewed. A “data subject” is any person whose personal data is being collected, held or processed. For European Union (EU) data subjects in the European Union, this use of the information is necessary for Orbis’s legitimate interests in understanding how the website and Orbis services are being used, to improve user experience, and to ensure network and information security.

By registering to attend the Conference, I hereby confirm the following: “I hereby grant Orbis Education the irrevocable right and permission to use photographs and/or video recordings of me on Orbis and other websites and in publications, promotional flyers, educational materials, derivative works, or for any other similar purpose without compensation to me.

I understand and agree that such photographs and/or video recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of Orbis.

I hereby release, acquit and forever discharge Orbis Education and its agents, officers and employees from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assigns and personal representatives.”
